Good morning, 

Suggestion: You may want to consider asking the applicants if he/she has received funds from PPP or EIDL. I have been asking my applicants because HQ may inquire how many applicants have received relief funds. If this happens, I don't want to have to back track and ask. In addition, if an applicant does not get accepted he/she many not want to answer the question and that will be lost data.

We are starting interviews next week.  I just want to ensure that I have the latest form (I could not find it on I-connect).  I have uploaded the Interview Form that we used in Y2019.  Can anyone confirm if this is the latest?

If not can you point to where I can access the latest, please?
